In defining a Sentinel Event, The Joint Commission states: “In support of its mission to continuously improve health care provided to the public, The Joint Commission reviews organizations’ activities in response to sentinel events. A sentinel event is any unexpected occurrence involving death or serious physical or psychological injury, or the risk thereof. Serious injuries specifically include a loss of limb or function. The phrase “or the risk thereof” includes any process variation for which a recurrence would carry a significant chance of a serious adverse outcome.” (Full PDF is here.) Multiple root causes in sentinel events represent a majority of cases reported to The Joint Commission. A full report covering sentinel events for 2004-2Q 2012 is available here.
Delay in treatment and elopements are high risk factors, and as acuities continue to increase in the emergency department so does the risk for these two areas of concern. Of course there are other factors that gauge your ED’s risk for sentinel events. We have those covered. Triage First is certain that the expert knowledge, resources, and education provided to our clients will Make it Better and put risk for sentinel events at the bottom of your concerns.
